Community Hospice & Adult Home Health Nurse

Full Time: Nights / On-Call

Schedule: On call from 4:30pm - 8am. 3 days one week (Wed, Thu, Fri) / 4 days other week (Wed, Thu, Sat, Sun)

External candidates are eligible for a one-time signing bonus of $7,000 (prorated for part time roles). The bonus will be paid out as a $3,500.00 pay adjustment in the first pay check and the remaining $3,500.00 will be paid upon completion of six months of satisfactory service.

The Community Hospice and Adult Home Health teams are looking for a full time night Nurse to support clients in the home setting. This position will cover overnight call for home-based Hospice and Adult Home Health clients.

The Nurse will provide a full range of skilled nursing care to patients in a home care setting, under moderate to minimal clinical supervision, with a focus on patient education and palliative care to meet the physical and emotional needs of patients, while educating and providing support to families.

Experience:
* Minimum or one or more years of directly related home or public health or hospice nursing experience

License Requirements:
* Current Vermont Nursing License

Performance Standards:
* Provide a full range of skilled nursing care to terminally ill patients in their homes, under moderate to minimal personal and procedural clinical supervision, including: ongoing assessments of current health status; venipuncture, catheter changes, care of central lines, and similar technical procedures; administering of medications and medication set-up; changing dressings and assessing wounds; teaching patients about their diseases, medications, diets and nutritional needs, physical activities; special terminal care and comfort measures; post-mortem care; information/advising clients about, referring them to, and assisting them in dealing with various government and community programs and services available to them
* Provide emotional support and general companionship to patients and their families and other caregivers, before and after death, as appropriate to individual circumstances
