Seeking LPNs to join our interdisciplinary team providing skilled nursing services to clients in their home. We offer an intensive mentoring program upon hire and ongoing education. We have a variety of schedules available to help you have a work/life balance including: part-time, full-time, and various baylor positions.

The Community Health Nurse, LPN will be responsible for the delivery of basic nursing care to patients in a home care settings, under minimal clinical supervision, as directed by a Registered Nurse, and in accordance with an established nursing plan of care.

Qualifications

• Accredited School of Nursing LPN Program
• Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience
• Current LPN licensure or compact licensure recognized by the State of Vermont required
