UVM Home Health & Hospice is seeking a skilled and dedicated High Tech LPN to join our growing team. We have a variety of schedules to support work life balance!

In this role, you will:

  • Develop long term, meaningful relationships with the patients and their families
  • Provide high-tech nursing care to technology-dependent patients in their homes
  • Collaborate with the healthcare team to ensure comprehensive patient care
  • Perform assessments, administer medications, and educate patients and families
  • Utilize innovative technology to monitor and manage complex medical conditions

We are looking for someone who:

  • Holds a current LPN license in Vermont
  • Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience
  • Previous home care experience a plus
  • Possesses strong clinical skills and a commitment to patient-centered care
  • Demonstrates excellent communication, collaboration, and critical thinking skills
